Individual Sessions

Offering office and Telehealth/virtual sessions.

Self-Pay or Out of Network Insurance Rates:

The initial diagnostic assessment (1-2 appointments) fee: $175/per session

Individual sessions (50-60 minutes): $150/per session

Please note: Some insurances will not cover Telehealth/Virtual sessions if out-of-network.

I cannot guarantee that insurance will reimburse services.

Accepted Insurances:

• BlueCross & BlueShield of IL PPO plans

• United Healthcare/Optum/UMR

You have the right to choose if you want to use your insurance or pay privately.

GOOD FAITH ESTIMATE

Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to provide a good faith estimate of expected charges for items and services to individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing, upon request or at the time of scheduling health care items and services.

You will receive a “Good Faith Estimate” of expected charges explaining how much medical care will cost when you request services from me, if you do not have/or plan to use insurance.
